This can happen multiple times in a day but it most commonly it happens in Conflict. I've had it happen once or twice after Shb launch in PVE content.

If audio cuts out for some reason during gameplay, the game doesn't try to re-initialise it. So uh... you'll need to fix whatever it causing this, cause the game won't recover without a restart.

Before this patch, it had only happened to me one time during an msq roulette about a year ago.
