I suspect each of them only drops with specific criteria, but what that is I am not sure. I mostly do strongholds in hard/challenging.

For whatever reason the memento won't drop trophies if your in a pre-WT map session. It also won't work properly if your a 31-39 leveling up through wony without a level 40 in your session.
