Ive killed 2 megs in the fog. Neither were the ghost. Both times I had been chased by the meg for a bit before the fog and it went away. So only tip I can offer is if you are heading to fog and a meg comes up kill it because im pretty sure that same meg will just keep spawning on you.

It's real, but it's spawn chance is in the single digits (it's either 1% or 3%, can't recall). Just to give you an idea, he was added with the Shrouded Spoils update, to this day, no one has unlocked the "Legendary Hunter of The Shrouded Ghost" commendation.
